H1N1 Vaccination Program Pricing.

Affiliated Physicians is pleased to offer the following pricing structure for the administration of the 2009/10 H1N1 vaccination program.

Current clients will notice that the pricing structure is different from past years. The new pricing structure is due to the limited availability and high demand associated with the H1N1 vaccine.

Pricing:

Company will be responsible to pay for a minimum of 80% of the requested number of shots.

• Example:
  If you request 120 shots, your minimum charge will be 120 X 80% X $17.00 = $1,632.00

Due to the short supply of H1N1 vaccine, Affiliated Physicians will only bring
the amount of vaccine requested to each program.

With each 25 shots requested, 1 hour of Nursing time will be included.

• Example:
  If you request 75 shots, 3 hours of nurse time will be included at NO additional cost.

For companies that request additional nursing time, the company will be charged an additional $60 per hour per nurse
