Affiliated Physicians is now scheduling H1N1 vaccination programs.

New York, New Jersey & Connecticut:

New Jersey, New York State (including New York City) and Connecticut have removed any restrictions concerning H1N1 vaccine. All individuals are now allowed to receive the H1N1 vaccine.

Other States:

Affiliated Physicians has the resources to administer H1N1 vaccination programs in other states.

Current vaccine availability and restrictions vary from state to state. Upon scheduling, Affiliated Physicians will notify you if H1N1 vaccination programs are currently available in your state.

Pricing:

Number of Shots Administered	Price Per Shot
1-35*	$20.00	* A minimum of 35 shots will be charged per program
36-100	$18.00
101+	$17.00

Company will be responsible to pay for a minimum of 80% of the requested number of shots.
Due to the short supply of H1N1 vaccine, Affiliated Physicians will only bring the amount of vaccine requested to each program.
With each 25 shots requested, 1 hour of nursing time will be included.
