Participant registration includes the collection and/or verification of employee demographic information, program scheduling, and the distribution of the necessary program paperwork.
Affiliated Physicians has created a proprietary online registration system that is made available to all clients. The online registration process is:
- Participants will be given a link to a website with a unique username and password for our clients
- AP will customize the website to our clients’s requirements
- Each participant will enter their basic demographic information
- Employee information can be verified against an eligibility list to be provided by our clients
- Each participant will be able to select the date and time that works best for them
- Participant will download the necessary program paperwork
- Participants receive an email with their appointment information

All participants must complete the following paperwork:
- Education Acknowledgement: Confirms that employees have read or viewed educational material and understand when to use antivirals
- HIPAA Privacy Notice: Standard HIPAA form which includes authorization for AP to release information regarding participation to our clients.
- Health History Form: Participants are given a health history form to complete prior to their appointment to expedite their health assessment.
